A HISTORY OF TETRAPLEGY DUE TO ISOLATED RETROPHARYGEAL ABSSES IN AN ADULT PATIENT

Abstract
Retropharyngeal abscess is a serious deep neck infection that is observed less frequently in adult patients and may cause significant side effects and even death due to its location. It may develop due to acute and chronic causes. These can frequently be observed due to respiratory tract, tonsil and dental infections, foreign bodies and trauma after interventional procedures. Patients may present with serious findings in this clinical picture, which occurs as a result of infection, suppuration and abscessation of the lymph nodes in the retropharyngeal region due to various reasons. Difficulty in breathing, difficulty in swallowing, limitation and pain in neck movements, and meningitis-like findings (nuchal rigidity, confusion, fever, paresis in arms and legs) may be observed in progressive cases. In this case report, we will describe the emergency department examination, diagnosis, surgery and follow-up of a 47-year-old male patient with no known comorbidities other than diabetes mellitus, who had a cold-like complaint one month ago and had been experiencing eating disturbance, neck pain, fever and confusion for several days.

Özet
Retrofaringeal apse erişkin hastalarda daha az sıklıkta gözlenen, bulunduğu bölge nedeniyle önemli yan etkilere yol açabilen, hatta ölüme neden olabilen ciddi bir derin boyun enfeksiyonudur. Akut ve kronik nedenlere bağlı gelişebilir. Bunlar sıklıkla solunum yolu, bademcik ve diş enfeksiyonları ile yabancı cisim, girişimsel işlemler sonrası travmaya bağlı gözlenebilir. Çeşitli nedenlerle retrofaringeal bölge lenf nodlarının enfekte olup süpürasyonu ile apseleşmesi sonucu oluşan bu klinik tabloda hastalar ciddi bulgularla başvurabilir. Solunum güçlüğü, yutma güçlüğü, boyun hareketlerinde kısıtlılık ve ağrı, ilerleyen olgularda menenjit benzeri bulgular (ense sertliği, bilinç bulanıklığı, ateş, kol ve bacaklarda parezi) görülebilir. Bu olgu sunumunda 47 yaşında, erkek, diyabet dışında bilinen ek hastalığı olmayan, bir ay önce soğuk algınlığı benzeri şikayeti olan, birkaç gündür yemek yemede bozulma, boyun ağrısı, ateş, bilinç bulanıklığı tarif edilen hastanın acil serviste muayenesi, tanı konulması ile beraber cerrahi ve takip sürecinden bahsedilecektir.
